When car insurance pays a medical claim do they subtract the cost of the injured parties insurance payout?

I’m just curious when someone gets into a car accident, lets say someone gets injured. They sue for medical expenses and in scenario 1 their insurance pays for it. Can they still file a claim for the full amount of treatment since they pay for that insurance and no law forces them to have insurance OR would the insurance payout the price remaining, if any?

How does that work?
Just want to know for idle knowledge.
By insurance paying for it, I mean the injured parties healh insurance.
